Here's how you can register...
Please go online to: https://www.harleystreetmedicalpractice.org.uk/
- On the left hand side of our website is a list of tabs. Please select the 'New Patients' tab. this will take you to further information regarding your registration.
- Please select the 'Form to register' tab.
- The Pre'Registration Form' can be submitted through our website. Please make sure to complete all the boxes with an asterisk.
- Your home address should be completed as your Stoke-on-Trent/Student Accomodation address. Please put where you have moved from as your previous address . (If this is completed incorrectly we won't be able to register you due to the address being out of our area).
- If you are a student that is coming from abroad, please make sure to fill in 'Date you first came to live in the UK'.
- Once you have finished your registration, please complete by clicking the 'send' button at the bottom of the form.
- If you are unable to complete the registration online/would prefer a paper form there is a link on the same tab which will allow you to print the registration off. We are happy to recieve these at Harley Street Medical Centre or via mail.
Alternatively, if you don't live on our Stoke-on-Trent campus, you can clik on the following link to find your nearest GP and register.
